High School Registration

▪ School hours are 8:00 A.M. – 2:37 P.M.

▪ Lodi High School Student are required to wear a NAVY BLUE uniform/polo shirt with khaki pants/shorts

o The embroidered logo on the uniform is recommended but not required.

o Non-embroidered uniforms can be purchased at any retailer as long as they match the color requirements:

Returning to the district? If your child was enrolled in a Lodi public school in the past and is now returning, skip to STEP 2. Former students do not need a new pre-registration application to be re-enrolled.

New to the district? If this is your child's FIRST time in the district, follow STEPS 1 & 2:

Step 1: Submit a Pre-Registration Application

Step 2: Prepare the required documents for submission.

● The Registration Team will email the primary contact after receiving the submitted application from STEP 1. They will provide the required paperwork and further instruction via email.

● If you are returning to the district, please email the full name and DOB of the returning student to registration@lodi.k12.nj.us. The Registration Team will reply with the required paperwork and further instructions.

● Once all required documents and packets are submitted and reviewed, you will receive an email confirming you have completed the registration process.

 

Registration Requirements

1. Proof of Lodi residency (one of the following):

a. All pages of current lease with student's name listed as an occupant

b. Mortgage statement/deed/contract of sale

c. Residency affidavit: fully completed by the property owner and notarized.

2. Birth Certificate

3. Immunization Records

4. Current Physical Report

5. Parent/Guardian photo ID

6. 2 supporting proofs of address (i.e. cable bill, PSEG, phone bill)

7. Registration Packet (provided to you via email)

8. Nurse Packet (provided to you via email)

9. Previous School Transfer Card

10. Previous School Unofficial Transcripts

 

If you are unable to provide ANY proof of residency, please contact the District's McKinney-Vento Coordinator, Amanda Serrano, by phone 973-778-5053 or email amanda.serrano@lodi.k12.nj.us.

You will not be asked for any information or document protected from disclosure by law, or pertaining to criteria which are not legitimate bases for determining eligibility to attend school. You may voluntarily disclose any document or information you believe will help establish that the student meets the requirements of law for entitlement to attend school in the district, but we may not, directly or indirectly, require or request:

• Income tax returns

• Documentation/information relating to citizenship or immigration/visa status, unless the student holds or is applying for an F-1 visa

• Documentation/information relating to compliance with local housing ordinances or conditions of tenancy

• Social security numbers
